By clicking the "Start Request" button below, you will begin the testing request form, which is immediately followed by our payment page. We are an approved Step Up for Students direct provider through EMA, and you will have the option to choose this as your payment method. If you will be private paying, you will be forwarded to PayPal to submit your payment at the time of form submission. 

You must complete a separate enrollment form for each student you wish to request testing for. Our current rates are:

Standard Delivery: $60 per student; Rush Delivery: $80 per student

We offer year-round testing! You are welcome to submit your testing request any day of the year. 

With standard delivery, we typically deliver your test license within 5 business days of the evaluation request and return your completed evaluation form within 5 business days of the test submission. With rush delivery, we send your achievement test license within 2 business days of the evaluation request and return your completed evaluation form within 3 business days of the test submission.

Our online enrollment form is easy, secure, and offers HIPAA-level security. Please make sure you have the following information available before proceeding to the request form. These will all be required to submit the form:

• Demographic information for you and your child

-  Full names, student's date of birth, email address, phone number

• Student's primary grade level

• Valid payment method (EMA direct pay -or- PayPal, Visa, Mastercard, Discover, or American Express)
